You may retrieve your clinical visit summary via the ONS patient portal. To obtain copies of additional medical record information and X-ray or MRI images, please utilize the options below. Please note, records will not be released without a request and authorization.

Charges

You will not be charged for records accessed via your patient portal. You may download your clinical visit summary (MRI/X-ray images not included) through the ONS patient portal. Login to your portal account here and click the Health Records icon.

You will not be charged for records sent directly to a healthcare provider for continuing care.

For all other requests, payment is required prior to releasing your records. An email invoice will be sent to your email address on file.

• For medical records (MRI/X-ray images not included) sent by US mail, the CT state-regulated rate is $0.65/page (maximum charge of $6.50).
• For MRI/X-ray images sent electronically via a HIPAA secure link, the fee is $15.
• For MRI/X-ray images copied to a CD and sent by US mail, the fee is $25.
