Medical Records & Imaging Requests

Orthopaedic & Neurosurgery Specialists (ONS) has partnered with HealthMark Group to ensure the accurate and timely completion of medical record requests. Please follow the instructions below to submit your request. We have a separate form below to submit requests for X-rays, MRIs and CT scans. Please note, records will not be released without a request and authorization.

Medical Records Request

To submit a request, please visit HealthMark’s Request Manager by clicking the button below. Once logged in, select “Submit Request” from the menu and fill in all required fields to authorize HealthMark directly. If you need to confirm your applicable ONS practice address, please visit our locations page. Your request will then be processed, and you will receive an email notification once it is complete and available for download. If you have any questions, please call HealthMark at (800) 659-4035.
