Medical Records & Imaging Requests

Orthopaedic & Neurosurgery Specialists (ONS) has partnered with HealthMark Group to ensure the accurate and timely completion of medical record requests.

Please follow the instructions below to submit your request. There are two separate request types:

1. Medical Records Request (Reports & Notes)
  • Office visit notes
  • Operative reports
  • Test results
  • Billing records
  • General medical records
  • Imaging reports (X-ray, MRI, CT scan)
    • Does NOT contain any image files
2. Imaging Request (Scans & Image Files)
  • X-rays
  • MRI
  • CT scans

1. Medical Records Request

To submit a request, please visit HealthMark’s Request Manager by clicking the button below. Once logged in, select “Submit Request” from the menu and fill in all required fields to authorize HealthMark directly. If you need to confirm your applicable ONS practice address, please visit our locations page. Your request will then be processed, and you will receive an email notification once it is complete and available for download. If you have any questions, please call HealthMark at (800) 659-4035.
