Medical Records & Imaging Requests

Orthopaedic & Neurosurgery Specialists (ONS) has partnered with MRO to support the accurate and timely processing of medical records requests.

Before You Submit A Request

A $6.50 fee applies to each medical records or imaging request.

Need medical records only? You can access your medical records at no cost through the ONS Patient Portal. Please note that actual X-ray and MRI images are not available through the Patient Portal.

Choose the type of request you need below:

1. Medical Records Request

For copies of:

  • Office visit notes
  • Operative reports
  • Test results
  • Billing records
  • General medical records
  • Imaging reports (X-ray, MRI, CT scan)
    • Does NOT contain any image files

Medical records requests are typically processed within 3 days.
