If you are a new patient or if it has been more than a year since your last visit to ONS, please download and complete the registration form and medical history form and bring them to your appointment. If you bring your completed forms with you, you do not need to come earlier than your scheduled appointment to do paperwork. Otherwise, please arrive 15 minutes prior to your appointment to complete the forms.
